This Forms area will be used for for parents and students to review and download various forms required by the State of Illinois, the Archdiocese of Chicago and/or St. Elizabeth of the Trinity School.

We have divided the forms into four sections – General Informational Forms, Required Forms, Optional Forms (e.g. Milk Fees & Order, EDP) and Athletics Forms .All forms are in pdf format .

PLEASE have all 2025-26 School Year forms in before August 1st!

Forms can be dropped off at the office  between 8:00 am and 3:00 pm Monday through Friday (8:00 am – 2:00 pm M-Th during summer hours).

You can always drop them in the Parish Drop Box – just include them in an envelope marked “Attention School Office”.
